Position Highlights: Quality of Life: Where career opportunities and quality of life converge Advancement: Strong orientation program, generous tuition allowance and career development Office Hours: Monday - Friday Under the direction of the Director of Quality, support our primary care practices in achieving success across payer quality incentive programs, including Medicare Advantage Stars, Medicaid quality programs, and commercial pay‑for‑performance initiatives. Serve as subject matter expert for payer quality incentive programs, including program requirements, measure specifications, reporting cycles, and financial impact Monitor, track, and trend practice performance across all quality incentive programs to ensure timely improvement and gap closure Coordinate and support practice-level quality improvement initiatives focusing on preventive care, chronic disease management, and patient experience Partner with primary care practices to support workflows that improve quality performance (e.g., outreach, closing care gaps, documentation best practices) Provide education on quality measure definitions and documentation requirements Collaborate with practice managers and our care coordination team to drive post discharge and social needs improvement Interfaces data from external facilities/organization for reporting purposes; develops support tools needed to obtain information or statistics vital to Quality Improvement efforts Develop and implement performance dashboards and actionable reports to guide practice-level interventions for Quality Improvement Manually and electronically provide EMR data in support of contractual and incentive programs Collaborate with clinical and IT staff to support defined business requirements Analyze data, prepare and present performance summaries to leadership, clinicians, and care teams Works with external Business Partners (i.e. Payers, ACO, CMS) to facilitate receipt and delivery of quality data Maintain up‑to‑date knowledge of quality measures and payer program requirements Maintains consistency and integrity of data collection and storage Performs other duties as assigned

Requirements

  • Associates Degree required
  • Minimum of five (5) years progressively responsible related work experience, including knowledge of research methods, basic statistics, business intelligence applications, and data analysis
  • Excellent analytical abilities and good problem-solving skills, required
  • Exceptional computer skills required, specifically reporting metrics
  • Proficiency with Microsoft Office suite, including strong Excel skills
  • Strong verbal and written communication skills, required.
  • Detail oriented, excellent follow up.
  • Ability to multi-task in a fast-paced environment.
  • Must be service oriented, quick learner, team player

Nice To Haves

  • Bachelors Degree preferred
  • Clinical/Healthcare experience, preferred.
  • EMR and Billing/Claims familiarity, preferred.
  • NCQA Patient-Centered Medical Home (PCMH) familiarity, preferred.
