About The Position

We are seeking a compassionate and detail-oriented Quality Coordinator (TCM & Case Management) to join our Quality Department. In this role, you will serve as the first point of contact for patients following hospital discharge, ensuring they receive timely, coordinated, and comprehensive post-discharge care. The Quality Coordinator plays a critical role in reducing readmissions, identifying unmet medical needs, closing care gaps, and supporting patients through their transition from hospital to home.

Requirements

  • High school diploma or equivalent required, higher education in health care preferred including medical assistant training or LPN
  • A minimum of 2 years experience in a clinical setting
  • Autonomous and demonstrates a high level of motivation and flexibility
  • Highly organized with ability to keep accurate notes and records
  • Proficient in Microsoft Word, Excel, Outlook email and billing software: Athena
  • Knowledgeable of ICD-10 and CPT coding
  • Excellent verbal/written communication skills
  • Leadership capabilities

Nice To Haves

  • Experience in case management, care coordination, or healthcare administration preferred
  • Strong communication and organizational skills
  • Ability to manage multiple tasks and meet time-sensitive deadlines
  • Familiarity with electronic medical records (EMR) systems preferred
  • Knowledge of post-discharge workflows, TCM processes, and healthcare coordination is a plus

Responsibilities

  • Contact patients within 48 hours of hospital discharge to initiate the follow-up process
  • Schedule follow-up appointments within 14 days of ER visits or hospital stays
  • Confirm and coordinate post-discharge care arrangements, including: Home readiness Durable Medical Equipment (DME) needs Specialist referrals
  • Document medication changes, discrepancies, or patient concerns during follow-up
  • Assist patients with: Coordinating or contacting Home Health Care (HHC) services Arranging transportation for follow-up appointments when needed
  • Ensure all patient communications are accurately documented and recorded
  • Collaborate with internal clinical and administrative teams to close care gaps and promote continuity of care
  • Identify and escalate urgent patient needs appropriately
