The Quality Care Coordinator (QCC) plays a crucial role in supporting the providers and leadership team in achieving high levels of member compliance with annual goals related to HEDIS, IHA, and STAR measures, as well as facilitating Annual Wellness Visits. The QCC utilizes Cozeva and other quality platforms to enter data derived from medical record reviews, document conversations with members or healthcare providers, and analyze data from various reports. This position is integral in reviewing patient due reports from Cozeva or similar platforms, organizing the data for effective member outreach, and ensuring that patients receive the necessary information and support to enhance their healthcare experience. In this role, the QCC is responsible for a variety of outreach activities, including mailing patient due letters or educational flyers, making calls to patients and healthcare providers, creating referrals, and conducting warm-handoffs. The QCC serves as a professional representative, striving to enhance patient satisfaction through effective communication and education regarding quality measures that are due. The position also involves providing general education related to these quality measures during both outbound and inbound calls with members.
