About The Position

Iowa Home Care is seeking a skilled QAPI Nurse (RN) to serve as a key member of the Quality Assurance and Performance Improvement (QAPI) team. This role supports agency quality initiatives, regulatory compliance, and clinical excellence across the continuum of home health care. This is an on-site position of Monday-Friday 8am – 5pm at our West Des Moines location. Benefits: Health, Dental, Vision, and Life Insurance Short-term and Long Term Disability plans Flex-Spending Employee Assistance Program Excellent 401K with Employer Match PTO and Paid Holidays Wellness Program to include onsite gym Continuing Education opportunities Key Responsibilities Serve as a leader and role model for teamwork, service excellence, and professional practice Support the QAPI program and assist with Performance Improvement Plans (PIPs) as assigned Ensure 100% compliance with clinically based Medicare Conditions of Participation Review and educate staff on SOC, ROC, transfers, and discharges Ensure appropriate care planning for complex patients Analyze hospital transfers and provide education to reduce readmissions Track, review, and ensure timely completion of OASIS and comprehensive assessments Review clinical documentation (OASIS, orders, plans of care) for accuracy, timeliness, and compliance Provide ongoing education related to clinical, regulatory, and performance improvement standards Serve as a clinical resource to all staff and assist managers as directed Qualifications Current RN licensure 3+ years of clinical nursing experience ~home health preferred Strong knowledge of Medicare Conditions of Participation and OASIS Experience with clinical documentation review and compliance Strong communication, organization, and critical-thinking skills COS-C certification preferred Why Choose Iowa Home Care? Requirements

  • Current RN licensure
  • 3+ years of clinical nursing experience ~home health preferred
  • Strong knowledge of Medicare Conditions of Participation and OASIS
  • Experience with clinical documentation review and compliance
  • Strong communication, organization, and critical-thinking skills

Nice To Haves

  • COS-C certification preferred
