About The Position

The Quality Associate is responsible for supporting evolving population health initiatives designed to improve the quality of care provided to Stamford Health Medical Group patients. Reporting to the Quality Manager, the Quality Associate manages pre-visit planning for assigned primary care and OB/GYN practices. The Quality Associate is responsible for reviewing and updating patient charts in advance of annual care visits to facilitate the best possible care and patient experience. Additionally, the Quality Associate serves as a subject matter expert on population health programs including the Merit-Based Incentive Payment System (MIPS), Patient-Centered Medical Home (PCMH) and Healthcare Effectiveness Data and Information Set (HEDIS). The Quality Associate is also responsible for supporting day-to-day activities of value-based care programs. The Quality Associate provides education, training, and program support to providers and regularly identifies and closes gaps in care through outreach to providers, practice managers, patients, and community resources. Schedule - Monday - Friday 40 hours per week onsite in Stamford, CT. Potential to work 1 day remotely after probationary period.

Requirements

  • Bachelor’s degree required.
  • Two or more years of related professional experience in a medical group setting is strongly preferred.
  • Experience with electronic health records (EHRs) is required. Experience with eClinicalWorks is preferred.
  • Working knowledge of population health programs including Merit-Based Incentive Payment System (MIPS), Patient-Centered Medical Home (PCMH) and Healthcare Effectiveness Data and Information Set (HEDIS) is preferred.
  • Proficient in Microsoft Office: Excel, Word, and PowerPoint.
  • Strong written and verbal communication skills.
  • Superior organizational and interpersonal skills.
  • Detail and process orientated with a focus on efficiency.

Responsibilities

  • Responsible for managing pre-visit planning activities for assigned primary care and OB/GYN providers. Planning activities include but are not limited to lab and imaging tracking.
  • Facilitates quality program education and training with providers and staff to ensure clear understanding of measure definitions, workflows, and data capture. Assesses challenges related to the electronic health record (EHR) and workflows and provides recommendations for improved performance.
  • Facilitates day-to-day activities for value based care programs with insurance companies.
  • Serves as a subject matter expert on programs including MIPS, PCMH, value-based care programs and HEDIS.
  • Supports the monitoring of MIPS performance and assists in the preparation of the annual data submission to the Centers for Medicare and Medicaid Services (CMS). Supports quality improvement initiatives across the medical group as assigned including patient experience improvement.
  • Provides training for providers, clinical staff and front desk staff related to quality programs including new employee orientation.
  • Prospectively and retrospectively works to identify and close gaps in care through communication with providers, practices, patients and community resources.
  • Participates in data validation for EHR reports as required.
  • Develops strong relationships with providers, practice managers, and medical assistants.
  • Attends and participates in quality team meetings.
