Provide quality assurance for the Strong Medicaid Health Home Care Management program. With the goal being to deliver high quality care to health home program participants and families, the Quality & Safety Specialist II is dedicated to achieving operational excellence and compliance with Health Home standards and quality guidelines, including but not limited to compliance with NYS DOH Health Home Program. Requirements

  • Bachelor's degree in Healthcare Administration or a related field required
  • 3 years of healthcare experience or equivalent combination of education and experience required
  • Strong project management skills, including organization of individual and team work prioritization; problem solving; adaptability; flexibility; and attention to detail preferred
  • Proficiency in data analytics
  • Ability to define and drive own work as well as carry out directives from others
  • Exceptional judgment and ability to learn the needs of different stakeholders
  • Instinctive capability to foster an inclusive, collaborative work environment
  • Excellent verbal, written and interpersonal skills; and Proficiency with Microsoft Office programs (Outlook, Word, Excel, Access and Powerpoint) and ability to learn new software as needed (Netsmart, eRecord)
  • This position requires an annual re-certification in HIPAA awareness and annual renewal of an iCARE Commitment contract.

Nice To Haves

  • Graduate degree in Business, Public Health, or Health Administration preferred
  • Previous work experience in clinical setting, population health initiatives or care management highly desired

Responsibilities

  • Auditing and Reporting Develops auditing and reporting processes to meet the needs of the NYS DOH Health Home Program, the lead Health Homes, the Care Management Agency, and the practice stakeholders.
  • Develops and implements operational, quality, and compliance reporting.
  • Communicates progress to Care Managers, Team Leaders, and practice stakeholders utilizing data visualization.
  • In coordination with the Financial Specialist and Health Home Director, reviews financial metrics and streamlines feedback to Care Managers, Team Leaders, and practice stakeholders Setting Quality Standards
  • Gathers feedback from leadership and stakeholders to identify performance benchmarks of importance, informed by NYS DOH Health Home Program requirements and internal Care Management Agency goals for the program
  • Incorporates standards into existing and new workflows and communicates to team members routinely to develop common understanding and expectations for Care Managers, Team Leaders, and practice stakeholders.
  • In collaboration with the Director of Health Home Care Management, reviews and considers for inclusion additional quality measures as they arise from NYS DOH Health Home Program, value-based payer contracts or other upstream enterprise initiatives that health home may impact. Process Development and Optimization
  • Participates in team development of processes to support Care Manager and team member workflows
  • Monitors and evaluates results of critical program aspects; identifies gaps for improvement
  • Provides cross-coverage in absence of Referral Coordinator
  • Understands the organizational visions and expectations of service excellence
  • Performs other responsibilities and projects as assigned
