The position involves delivering exceptional and individualized care to residents as determined by each resident's care plan. The role requires reporting changes in resident health status to the clinical team and connecting with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience. Continuous learning and development opportunities are encouraged through attendance and participation in educational seminars, staff meetings, and other learning experiences. Compliance with all rules and regulations regarding confidentiality and privacy of resident information is mandatory. Additional duties may be assigned as needed.

  • High School diploma or GED equivalent required
  • Must hold certification indicating successful completion of Qualified Medication Aide (QMA) program in the State of Indiana
