About The Position

The QIDP is supervised by the Director of ICF, is responsible for managing the day-to-day operations of multiple ICF group homes. This includes responsibility for developing Individual Support Plans in coordination with the interdisciplinary team as well as direct supervision of Home Coordinators and Direct Support Professionals, working in partnership with the R.N. Area Manager. This position reports directly to the ICF/IDD Director. All services are to be provided in a manner consistent with SRVS values, mission, and principles as well as in accordance with policies, procedures, practices and licensing standards.

Requirements

  • Minimum of a bachelor's degree in a related area
  • Previous experience working with persons with intellectual and developmental disabilities, minimum of five years

Responsibilities

  • Participates in the interviewing and screening of all individuals being considered for placement in the ICF/ID home.
  • Writes the individual Support Plan (ISP) at annual staffing following completion of all assessments and maintains other documentation in accordance with agency and funding requirements (monthly and quarterly progress of Service Recipients and ongoing progress notes).
  • Assures that all the information in the Service Recipient's Health Passport is up to date.
  • Directly supervises (coaching, training, monitoring, giving feedback, evaluating and documenting performance) Home Coordinators and Direct Support Staff with the expectation of compliance with agency policies and procedures and provision of services in accordance with job descriptions and implementation of Individual Service Plans.
  • Works in partnership with the R.N. Area Manager in monitoring services provided by the interdisciplinary team with the goal of reinforcing positive job performance and continuous improvement.
  • Interacts regularly and frequently with parents/conservators to provide current information about Service Recipients, answer questions, resolve problems, and plan for participation in the annual Individual Service Plan (ISP) staffing and Individual Education Plan (IEP) team meetings.
  • Acts as liaison with community resources and coordinates the services of consultants and professional services which are or might be used by Service Recipients.
  • Develops and oversees Community Participation Monthly Calendars with the SRVS
  • Recreation Therapy Consultant and Home Coordinator and monitors to assure activities are individualized according to the Individual Service Plan.
  • Works with the ICF Secretary to correct any errors or oversights in Service Recipients' medical insurance coverage information.
  • Manages Service Recipients' trust fund accounts and assists in budgeting for personal expenditures.
  • In collaboration with the ICF Director, monitors the appropriateness and budgetary compliance of expenses incurred in maintaining home operations in the areas of food and house supplies (purchases by the Home Coordinators).
  • Serves on a rotating call schedule along with RN Area Managers and other QIDPs.
  • Attends all scheduled and mandatory meetings
  • Maintain confidential client files, budget, program documentation
  • Prepare and present reports
  • Manage and distribute resources appropriately
  • Serves on agency committees as assigned
  • Works cooperatively with supervisors, staff, and families, other Program Specialists
  • Demonstrates respect for all agency clientele
  • Performs other duties as assigned
