About The Position

CTF Illinois is seeking a dedicated and organized QIDP / Case Manager to support individuals with developmental disabilities in their Community Day Services Program at the Tinley Park location. The role is full-time, Monday–Friday during daytime hours, with no holidays or weekends. The organization is dedicated to helping people with intellectual and developmental disabilities reach their full potential through high-quality, person-centered programs and services. Requirements

  • Bachelor’s degree in a human services-related field
  • Must meet certification and training requirements for QIDP
  • At least 1 year of experience working with individuals with developmental disabilities
  • Valid driver’s license with an acceptable driving record
  • Strong organizational skills and ability to work independently

Nice To Haves

  • QMRP/QIDP certification preferred
  • Previous DSP training a plus

Responsibilities

  • Develop and implement individualized strategies based on each person’s goals, preferences, and personal plan
  • Coordinate services by maintaining regular contact with individuals on your caseload
  • Participate in discovery and interdisciplinary team meetings
  • Complete required assessments, documentation, and consents
  • Monitor progress and outcomes through monthly reviews and reporting
