About The Position

The Qualified Intellectual Disabilities Professional (QIDP) is responsible for supervising the development and implementation of each client's Individual Habilitation Plan (IHP) by integrating services according to their Individual Service Plan (ISP). This role ensures the continuity and quality of client services, facilitating their participation in community outings and recreational activities, and promoting individual rights and healthy lifestyle choices.

Requirements

  • Bachelor's degree in a field related to human services or developmental disabilities.
  • Minimum of 2 years of direct experience in the care and training of persons with developmental disabilities.
  • Must meet State and Federal requirements as a Qualified Intellectual Disabilities Professional (QIDP).
  • Bi-lingual (English & Spanish).
  • Must be available to be fingerprinted and receive a criminal record clearance.

Responsibilities

  • Supervise all aspects of planning and implementing the Individual Habilitation Plan (Day Hab plan) for each client.
  • Facilitate client participation in community outings and recreational activities.
  • Assist in the DSP staff's orientation, training, and mentoring as required.
  • Monitor ISP implementation, including training direct support staff on plan implementation, writing monthly summaries, and updating/revising the plan as needed.
  • Assist individuals in accessing appropriate services and/or resources for recreational, educational, community, spiritual, health & wellness activities, and other life areas.
  • Promote individual rights; ensure individuals are informed of their rights.
  • Promote and provide support to individuals in making healthy lifestyle choices.
  • Participate in long and short-term program committees.
  • Attend required training.
  • Maintain open, frequent communication with management regarding concerns and general client status; consultants and interdisciplinary team members regarding client needs; and applicable school and adult day program personnel.
  • Act as liaison between staff, consultants, and day programs.
  • Work cooperatively with all interagency departments, respecting the established chain of command.
  • Participate in the Human rights /Incident review committee meetings.
  • Adhere to all agency policies and procedures. Follow service provision procedures as indicated in the agency Quality Assurance Program.
  • Perform additional duties as assigned.
