About The Position

A large, mission-driven workforce services organization supporting public and community-based programs is seeking a Qualified Health Professional - Level I . This role focuses on clinical and functional capacity assessments that support employment readiness, vocational planning, and disability-related determinations. This position is well suited for clinicians who thrive in structured, performance-driven environments and who value interdisciplinary collaboration, documentation accuracy, and public-sector impact. A hybrid work schedule is available.

Requirements

  • Active licensure or certification as one of the following: Licensed Clinical Social Worker (LCSW) Licensed Master Social Worker (LMSW) Certified Rehabilitation Counselor (CRC) Licensed Mental Health Counselor (LMHC) Other approved credentialed health professionals
  • Minimum four years of clinical assessment experience , including document review, bio-psychosocial assessments, structured interviewing, and disability-related evaluations
  • Experience working in fast-paced, metrics-driven, team-based environments
  • Strong customer engagement, communication, and interviewing skills
  • Training or applied experience with motivational interviewing and care management approaches
  • High attention to detail with strong organizational and documentation skills
  • Knowledge of regulatory and compliance requirements relevant to clinical assessment roles
  • Ability to work efficiently within electronic case management and documentation systems

  • Experience with publicly funded or government-supported programs , preferred

Responsibilities

  • Conduct initial and follow-up clinical assessments for referred individuals
  • Review medical, behavioral health, and related documentation using established protocols
  • Perform bio-psychosocial evaluations using approved tools and structured interviewing techniques
  • Apply standardized disability evaluation frameworks to determine functional capacity outcomes
  • Conduct mental health evaluations when indicated through assessment findings
  • Identify functional limitations and determine employability, rehabilitation, or further evaluation needs
  • Request or coordinate additional medical, behavioral health, or substance-use assessments as required
  • Coordinate with reviewing clinicians or physicians when further determination support is needed
  • Partner with engagement or outreach staff to ensure follow-up activities are completed
  • Compile assessment documentation and prepare required clinical reports
  • Obtain required client acknowledgements and signatures upon completion of assessments
  • Collaborate with service delivery teams to interpret assessment findings that impact care or service planning
  • Utilize motivational interviewing techniques to obtain accurate, comprehensive information
  • Participate in team meetings, trainings, and continuous process improvement initiatives
  • Adhere to all applicable policies, procedures, and regulatory requirements
