The position involves facilitating person-centered behavioral management plans tailored to the individual needs, goals, and desires of the person. The role includes providing ongoing training to direct care staff in collaboration with the Nurse Educator, reviewing the effectiveness of behavioral management plans, and ensuring accountability for the provision of supports and services. The individual will also be responsible for monitoring incidents, health concerns, and acting as the primary contact for the person and their representatives. Administrative duties include completing necessary documentation, maintaining accountability for habilitation requirements, and coordinating services with staff. The position requires effective case load management, timely responses to crises, and maintaining a respectful and compassionate approach towards consumers and their families. Participation in performance improvement activities and required training is also expected.
