About The Position

As a QMHS +3 Case Manager , you will play a vital role in supporting individuals and families on their mental health journey. In this role, your work will directly impact the emotional, psychological, and overall well -being of those we serve by ensuring each client receives personalized guidance, advocacy, and access to life -enhancing resources. Requirements

  • Bachelor's or master's degree, or a high school diploma/GED/Associate's degree.
  • At least three (3) years of relevant experience in the mental health sector.
  • Experience in roles such as mentorship, social skill -building, coping skill development, emotional wellness support, summer programs, after -school programs, or daycare.
  • Excellent communication and interpersonal skills.
  • Ability to thrive in a collaborative, team -oriented environment.
  • Successful completion of a background check and drug test.
  • Resume must clearly outline educational background and relevant experience.

Responsibilities

  • Provide comprehensive, family -centered case management services, including assessments, care planning, and ongoing coordination tailored to each client’s individual needs.
  • Develop and implement personalized service plans that reflect each client’s goals, strengths, and challenges, treating every individual with compassion and respect.
  • Collaborate closely with healthcare providers, families, and community partners to ensure holistic, client -focused care.
  • Connect clients and families to vital community resources and support networks that enhance stability and quality of life.
  • Monitor, document, and evaluate client progress, thoughtfully adjusting care plans to support continued growth and success.
  • Serve as a dedicated advocate for clients and their families, ensuring their voices are heard and their rights are protected.
  • Maintain consistent, supportive follow -ups to foster trust, encourage engagement, and reinforce each client’s sense of belonging and care.
