Performs outpatient competency restoration education, routine case management, skills training, medication training and support, and psychosocial rehabilitation services based in trauma informed and person driven care model to a caseload of individuals with legal issues related to outpatient competency restoration. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, legal, and other individualized services. Is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the plan for services to meet the individual’s identified needs in the community, home, legal system, and school. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. Position will work closely with criminal justice agencies. Goal is to link criminal justice involved individuals with appropriate mental health and substance use disorder services to reduce recidivism and restore competency while treating for serious mental health concerns to ensure individuals receive the services, they need to remain stable, functioning in the community, and regain competency when possible.
