Performs field and office based intensive social services with individuals aged 15-30, whom have been diagnosed with their first episode of psychosis within the last 2 years. All service delivery is based in a trauma informed and person driven care model. Works as part of an intensive team with a small caseload (20-30) with the goal of stabilizing individuals in the community and helping them and their families better understand psychosis. Performs supported employment, skills training, and psychosocial rehabilitation services based in trauma informed and person driven care model. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Provides person directed skills training and psychosocial rehab to develop coping and other skills needed to feel successful in their home and community, specializing in those related to employment. Must understand and apply principles of SAMSHA Supported Employment model to ensure fidelity requirements are met. Must work well with community partners to assist individuals with finding and maintaining employment opportunities. Must be up to date on all required training and specialized training for the Coordinated Specialty Care program.
