The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. The QIDP Coordinator/Director works under the direct supervision of the Assistant Director of Programs (ADOP) or equivalent and has the primary responsibility for providing leadership and directions to the Interdisciplinary Team (IDT) to assure the delivery of quality program services that promotes dignity and respect for the individuals served. Plans, assigns, supervises, and monitors the work of their assigned IDT members. Ensures that group home employees receive the appropriate orientation and training in their job duties. Performs managerial functions including the hiring and performance evaluation processes. The QIDP /Director is responsible for working cooperatively with other divisions and departments and for ensuring harmonious working relationships within his/her group homes among staff, family members and other group home employees. The QIDP / Director is responsible for planning and coordinating the use of all departmental resources. The QIDP / Director is responsible for training and coordination of QIDP services and data processing services. This position is required to manage a QIDP caseload. The QIDP / Director performs complex consultative serves and provides technical assistance to Interdisciplinary Team (IDT) members. Some flex time, overtime, evening and weekend work will be required.

Requirements

  • Knowledge of ICF/IID standards.
  • Knowledge of the Interdisciplinary Team (IDT) process.
  • Knowledge of people-centered planning.
  • Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities, including program design and collecting and analyzing data.
  • Knowledge of discipline specific plans and processes such as Positive Behavior Support Plans (PBSP) and Physical Nutritional Management Plans (PNMP).
  • Knowledge of the role and responsibilities of the QIDP at an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF-IID).
  • Knowledge of leadership and management tools.
  • Knowledge of education/training and active treatment methods, tools, techniques, and processes.
  • Ability to coordinate programmatic and clinical services and supports.
  • Ability to prepare and maintain records.
  • Ability to quickly adapt to change.
  • Ability to communicate verbally and in writing in a clear, effective, concise and constructive manner.
  • Ability to assess and solve problems.
  • Ability to work independently and organize/prioritize work, and to work cooperatively in a teamwork situation.
  • Ability to work harmoniously, assertively, and professionally with staff at all levels and with Legally Authorized Representatives (LARs).

  • Ability to solve problems, make decisions, and use good judgement when dealing with critical incidents.
  • Skill in providing on-the-job training to new staff and recurrent/refresher training to incumbent staff.
  • Skill in facilitating IDT meetings to ensure participation by all members and achieve desired outcomes and modeling facilitation for others.
  • Skill in using a personal computer to create documents and reports.
  • Skill in using electronic records systems and/or databases.
  • Bachelor’s degree from an accredited college or university with a major in psychology, sociology, social work, special education, rehabilitation counseling, or a related human services field such as therapeutic studies (e.g., music, art, occupational, physical therapy), recreation, and/or a combination of equivalent subject matter. For a related human services field, the transcript must be reviewed for applicable coursework that meets federal guidelines for QIDP designation.
  • At least three (3) years of experience working directly with people with intellectual disabilities or other developmental disabilities.

Nice To Haves

  • At least one (1) year of experience as a QIDP is preferred.
  • Experience training, leading the work of others and/or supervising others is preferred.

Responsibilities

  • Provides leadership and support to all Group Home employees to ensure the delivery of quality services that promotes dignity and respect for the individuals.
  • Provides oversight and monitoring of all QIDP related duties ensuring that professional practices are in compliance with ICF/IID.
  • Oversees and participates in the screening, interviewing, and selection of all Group Home staff.
  • Provides supervision, leadership, and support.
  • Monitors the performance of Group Home staff and coordinates the performance evaluation.
  • Develops and implements strategies to motivate, encourage, and equip staff.
  • Promotes harmonious working relationships among staff, family members, and other employees.
  • Develops, revises, and maintains policies/procedures relating to QIDP duties and the ISP development, implementation and monitoring process.
  • Ensures Group Home Staff are knowledgeable of and effectively implementing these practices.
  • Educates, encourages, and supports all staff through the coordination and oversight of departmental training efforts.
  • Performs complex monitoring, tracking, trending, and analysis of data regarding outcomes for individuals and IDT activities.
  • Participates in various committees and work groups designed to ensure individuals are receiving quality services and care.
  • Coordinates efforts to develop solutions and follows-up until problems are resolved.
  • Performs after-hours monitoring and other monitoring duties as assigned to ensure consistent program coordination and implementation.
  • Performs special projects and other duties as assigned.
  • Participates in mandatory training per HHS policies and procedures, HIPAA policies, state and federal rules and regulations, and ICF-IID regulations.
  • Actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
