About The Position

The You First team is seeking a dedicated Lifestyle Program Specialist to support their heart health initiative. This role involves working directly with program members to enhance their health and reduce the risk of cardiovascular disease through personalized support and resources. Member interaction is primarily conducted by phone and email. This position is funded by a cooperative agreement with the Centers for Disease Control and Prevention. It is a hybrid position based in Waterbury, VT. Strong attention to detail and a passion for improving the health and well-being of all Vermonters are necessary attributes for this position. This is a Limited Service position, which is non-tenured and authorized for a specific period of time, established for specially funded projects or programs.

Requirements

  • Three (3) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
  • OR Associate degree in public health, public administration, business administration, environmental science, emergency response, human services, social work, social sciences related fields or health-related fields and two (2) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
  • OR Bachelor’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields and one (1) year or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
  • OR Master’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields.
  • OR One (1) year or more of experience as Public Health Specialist I.
  • Candidates must pass any level of background investigation applicable to the position, including Vermont and/or national criminal record checks, as well as DMV and adult and child abuse registry checks.
  • Private means of transportation must be available for required travel.

  • Background in nutrition, health coaching, motivational interviewing, or physical activity.

Responsibilities

  • Review cardiovascular risk assessments (blood pressure, A1C, cholesterol, health history, and social determinants of health)
  • Utilize motivational interviewing techniques
  • Provide personalized health coaching to support lifestyle changes
  • Facilitate access to resources such as fresh produce (farmers markets/CSAs) and physical activity opportunities (walking programs, gym memberships, exercise classes)
  • Manage relationships with state vendors.
  • Perform documentation and administrative tasks
