The Public Health Nutritionist is responsible for advanced and complex professional public health nutrition and dietetic work in the Florida Department of Health in Duval County. This employee helps to plan, conduct, and monitor a generalized nutrition program in a designated area. Work is performed under the direct supervision of a Health Education Program Consultant. Provides services in the WIC program in accordance with the policies of the state WIC program and regulations as stated in DHM 150-24. Responsible for caseload management as set forth by the nutrition plan. Provide comprehensive nutrition and breastfeeding assessment for low-risk, high-risk, and medical high-risk WIC clients. Provide nutrition counseling with SMART goal setting and follow up with clients at future appointments. Document all counseling using the SOAP format in the FL-WISE system appropriately in accordance with department policy. Provide client-centered nutrition and breastfeeding education in individual and/or group settings to promote good eating habits, physical development and good health as needed. Conduct Medical Nutrition Therapy (MNT) to clients as prescribed by the medical care provider. Communicate with the interdisciplinary team to ensure continuity of care. Coordinate other services for clients and make referrals to appropriate agencies as needed. Eligibility Certify clients for participation in the WIC Program. Obtain client’s height, weight and hemoglobin as needed. Review Immunization records for all infants and children participating in the WIC Program and document appropriate information in the FL-WISE system. Formula Assess formula needs and make referrals for formula recommendations as appropriate. Assess formula amounts provided by the WIC program. Assist with obtaining up to date and accurate prescriptions from providers. Follow procedures for issuing and reconciling special formula including direct distribution and EBT. Ensuring formula inventories are accurate. Interns and Outreach Act as preceptors for future registered dietitian and dietetic interns: Pasco County DOH, Mayo Clinic UNF, and other participating internship programs. Provide interns with experiences in community nutrition education, case studies, calculations, medical nutrition therapy, and various diseases states and therapeutic diets. Familiarize interns with WIC program policies and procedures and educate on other community programs. Conduct outreach visits to other appropriate agencies, offices, and organizations to describe the benefits of the WIC Program, eligibility requirements and service locations. Participate in health fairs and provide nutrition information as needed. Plan, develop, and conduct education classes for the community in relation to Community Nutrition Programs. This includes presentations and speaking engagements in numerous settings such as schools, community agencies and organizations, churches, and faith-based organizations, health fairs, and corporations. Other Employee Duties Attend mandatory and scheduled training. Submit accurate and timely Employee Activity Record (EARs), time records, travel logs, monthly reports of activities and other reports are submitted to NCDP Administrative Office. Answer telephones, file records and perform travel in relation to above duties and for professional development. Participate in the FDOH in Duval County Nutrition and Chronic Disease Prevention Disaster Plan as required. Performs all other assigned duties as required.
