General Duties Include: Collaborates with state and local health department staff, healthcare providers, and community partners to strengthen diabetes prevention and management efforts across the service area. Serves as a key resource for individuals with diabetes, those at risk for diabetes, and healthcare professionals by providing education, tools, and guidance that support improved health outcomes. Facilitates evidence-based education programs that encourage healthy lifestyle changes among individuals and families affected by diabetes or prediabetes. Programs may include diabetes education classes, weight management and nutrition initiatives, cooking demonstrations, support groups, and other community education events. Coordinates Diabetes Prevention Program (DPP) activities to ensure compliance with National Diabetes Prevention Program (NDPP) standards and assists with preparation of required program reports.
