About The Position

Under limited supervision, provide management of positions, employees, and activities within the operations of the Community Health Division including management of the Population Health and Data Analytics Unit. This includes coordination of staff trainings, assignment and review of work, participation in interview and selection processes for new employees, making recommendation which impact salaries, job status, and disciplinary action, and evaluating performance. This position oversees and provides direct support, assistance, and guidance around public health data needs and techniques for public health data analysis. Additionally, this position manages all aspects of the Weld County Community Health Improvement Planning process, including lead writer of the Community Health Improvement Plan. The position will collaborate with other health agencies, organizations, business groups, municipal governments, community members, and key community leaders to coordinate the provision of population and public health services through coalitions and partnerships. Collaborates with community partners to identify and evaluate top health issues facing Weld County. Collaborates with Department leadership on quality improvement and strategic plan activities of WCDPHE and the Community Health Improvement Plan. Performs professional and administrative work including the planning, development, implementation, and project management of the Weld County Community Health Improvement Plan. Requirements

  • Required Education
  • Master's Degree from an accredited college or university in public health, health education, community health, evaluation, or related field and at least four (4) years of public health work experience including at least two (2) years of supervisory experience.
  • OR
  • Bachelor's Degree from an accredited college or university in public health, health education, community health, evaluation, or related field and at least five (5) years of public health work experience including at least two (2) years of supervisory experience.
  • An equivalent combination of education and work experience that satisfy the requirements of the position may be considered.
  • Skills and Abilities
  • Knowledge of principles and practices of population-based public health education, evidence-based prevention strategies, emerging disease trends, chronic disease prevention, and planning and evaluation.
  • Knowledge of electronic communication, social media, including word processing, managing files and records, designing forms, and other office procedures and terminology.
  • Ability to effectively present and communicate information, ideas, and data, verbally and in one-on-one, small group situations, and public forums as well as in writing routine reports and correspondence and sharing information with data graphics or visualizations.
  • Ability to supervise teams and coach staff as appropriate; ability to solve problems independently and conduct appropriate performance appraisals.
  • Ability to collaborate, work well in a team environment, work with a diverse group of people, establish and maintain effective working relationships with employees, professionals, other departments, agencies, and the public.
  • Ability to adapt to ever-changing work environment, use limited oversight with independent judgement, and respond to requests on tight timelines.
  • Must have the ability to be reached via a mobile phone and work outside of normal business hours as necessary, especially during emergencies and as needed by the Health Department Executive Director and Community Health Division Director.

  • Candidate must have a valid Driver’s License and Liability Insurance Upon Hire.
  • Candidate must pass a Motor Vehicle Record (MVR) evaluation and if hired, will be subject to continuous monthly MVR monitoring and random drug screens throughout employment.
  • Candidate must pass criminal background check prior to employment start date.
  • Pre-employment substance screening is required for this position.
  • This position is subject to a pre-employment drug screening.
  • Employment is contingent upon successfully passing the screening in accordance with Weld County policy.
  • This position is exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act and is therefore ineligible for overtime pay.

Responsibilities

  • Community Health Improvement Plan/ Thriving Weld Management, Facilitation, Implementation, and Reporting - 40%
  • Serve as Project Lead for the planning, development, and writing of Weld County’s Community Health Improvement Plan (CHIP).
  • Ensure plan meets minimum requirements of 25-1-505 CRS.
  • Oversee report timeline and progress.
  • Lead implementation and monitoring efforts of the CHIP after planning phases are complete and CHIP approved.
  • Lead development of annual work plans, performance metrics, reporting criteria, and other related processes and documents.
  • Oversee the planning, development, implementation, and facilitation of community-based strategies, interventions, and campaigns specific to CHIP priorities.
  • Collaborate with various partners, business and municipal leaders, and community groups to ensure the success of workplan deliverables.
  • Make regular progress reports, including the CHIP and other initiatives, to community partners, Health Department leadership, and Board of County Commissioners.
  • Lead, convene, and facilitate CHIP leadership team of external partners (known as Thriving Weld Steering Committee).
  • Organize, plan, schedule, and facilitate Thriving Weld quarterly meetings and Annual Summit.
  • Leadership, Strategy, Coordination, and Communication - 35%
  • Advance Division and Department goals related to Strategic Plan, quality improvement, and performance management.
  • Facilitate and provide leadership for workgroups implementing strategies to meet goals.
  • Analyze data, create reports, and make recommendations.
  • Assist, plan, and/or manage aspects of community health assessment projects such as communitywide surveys including preparing materials for approvals, grant proposals, cost estimates, and preparation of budgets and timelines.
  • Ability to interpret a variety of data findings and communicate results in written, oral, or graphic form.
  • Develop and present written and oral reports and presentations to diverse audiences that communicate necessary information to professional audiences, policymakers, and the public.
  • Identify problems, develop advanced solutions, and take a self-directed approach to solving problems.
  • Lead development and maintenance of evaluation plans to ensure effectiveness of health education and public health programs.
  • Make recommendations for future programming.
  • Identify and access funding for programs by planning, writing, and organizing approval of grant proposals.
  • Assist in identifying evidence-based, research-based, or promising practices for future programming.
  • Ability to work well in a team environment, ability to work with diverse groups of people and ability to establish and maintain effective working relationships with employees, professionals, other departments, agencies, and the public.

  • Supervision, Administration, Employee Development - 25%
  • Motivate, lead, mentor, coach, and supervise a team of health communication specialists to meet goals and objectives.
  • Plan and allocate resources to support health department needs; monitor budget expenditures for assigned programs and issue purchase orders and requests for program supplies and expenses.
  • Ability to train and/or coordinate the work of others, including students and staff.
  • Develop, communicate, and monitor work assignments.
  • Create job posts, conduct hiring interviews, and select candidates for job openings.
  • Conduct performance appraisals.
