About The Position

We are currently expanding our elite clinical team and are hiring for Full-Time, Part-Time, and PRN positions. To ensure professional stability and work-life balance, we require a 4-hour per week minimum for all staff. The Physical Therapist Assistant at Breakthrough Healthcare provides skilled intervention and treatment under the supervision of a Registered Physical Therapist. You will implement the physician-signed plan of care, monitoring patient progress and ensuring the highest standards of safety and compliance in the home set Key Responsibilities: Implementation: Deliver services exactly as planned and delegated by the supervising PT. Proactive Reporting: Observe and record reactions to treatment, reporting any condition changes immediately to the PT and Clinical Manager. Documentation Excellence: Complete clinical notes and progress reports according to strict agency timeframes. Community Coordination: Attend care conferences and participate in quality improvement programs to ensure seamless service Treat Clients: to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Observe, record, and report: the client’s reactions to treatment and any changes in the client’s condition to the PT and/or Clinical Manager. Collaborate: with the PT in assisting the physician in assessing the client’s functional level by applying appropriate tests. Assist in the development and revision of the plan of care. Complete: clinical notes and progress reports according to agency policy and time frames. Attend and participate: with care conferences to ensure coordination of care. Participate: in the agency’s quality improvement program. Keep abreast of physical therapy trends and knowledge. Participate with in-service programs.

Requirements

  • Licensed, registered, or certified as a Physical Therapist Assistant in the state of Missouri.
  • Graduate of an APTA-approved PTA curriculum.
  • Successful completion of the national examination for Physical Therapist Assistants.
  • Comfortable traveling to and performing in varied patient home environments.
  • Two years of solid experience as a PTA
