About The Position

A Physical Therapy Assistant is someone who provides coordinated physical therapy to patients of all age groups under the supervision of a licensed Physical Therapist. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.

Requirements

Physical Therapy Assistant, licensed per the state requirements
2 years of Home Health preferred
Good communication skills, assessment skills, and strong clinical documentation skills.
Ability to read, write, and speak the English language
Reliable transportation. Valid and current auto liability insurance

Responsibilities

Treats patients as directed by the Physical Therapist.
Records treatments administered in medical record.
Records clinical documentation according to accepted regulatory, corporate and professional guidelines.
Participates in weekly case conferences.
Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
Provides verbal or written patient status updates to supervising PT prior to reassessment and discharge visits, and as indicated by patient response to treatment.
Participates in in-service training programs.
Responsible for assuring patient safety.
Complies with state regulations and national standards as they apply to the practice of physical therapy.
All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
