About The Position

Ready to make a real difference in residents' lives? As a PT QMAP in Assisted Living, you'll be the go-to person for our residents' daily personal care needs—helping them live their best lives while staying true to our mission and values. You'll be a keen observer, spotting care needs before they become issues, and working hand-in-hand with our nursing team to keep everyone safe and healthy.

Requirements

  • Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
  • Must be able to read, write and speak the English language.
  • High school or Equivalent (GED)
  • One (1) year experience in some level of elder care preferred
  • Proficient computer skills preferred
  • Strong self-management and organization skills
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality

Responsibilities

  • Be a Personal Care Champion: Jump in to help residents with their daily activities—bathing, dressing, meals, laundry—with genuine attention to the details that matter (combed hair, tied shoes, fresh breath!).
  • Stay One Step Ahead: Anticipate what residents might need before they ask. Notice the little things—hygiene needs, broken eyeglasses, skin changes, missing hearing aids—and flag them to your supervisor right away.
  • Keep the Communication Flowing: Chat regularly with your supervisor and teammates to stay in the loop about resident needs and any concerns that pop up.
  • Be a Safety Guardian: Keep a watchful eye on residents' safety and well-being, and always report any suspicions of abuse, neglect, or misuse of resources as required.
  • Track the Changes: Notice and report any shifts in residents' health or status to your supervisor, and document their condition as directed.
  • Master Medication Management: Assist residents with their daily medications in line with state regulations and established procedures. Double-check that the right resident gets the right dose at the right time via the right route. When in doubt, loop in the RN/LPN or supervisor.
  • Keep Records Straight: Make sure all documentation happens promptly, including any medication refusals or treatment declines.
  • Restock and Refresh: Keep medication carts and storage areas stocked with supplies (gloves, sanitizer cups, etc.) and submit work orders for any repairs or replacements needed.
  • Show Up for the Team: Attend and actively participate in monthly staff meetings, bringing your insights to the table on resident and departmental needs.
  • Roll with It: Take on other duties as assigned—we're a flexible, collaborative team!
