Requirements
- Clinical credentials: PA certification with a genuine passion for geriatric medicine (prior geriatric experience preferred but not required)

- Mobility: Comfortable providing onsite care across multiple senior communities within the market

Responsibilities
- Provide comprehensive onsite primary care to seniors in assisted living and senior living communities
- Develop and execute personalized, patient-centered care plans that address the whole person
- Partner with our Medical Director to refine clinical policies, protocols, and guidelines as our practice evolves
- Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
- Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes
