Requirements

• Licensed Physical Therapist in the state of practice
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of experience preferred (acute care, outpatient, home health, or community health)
• Current CPR certification and up-to-date PPD or chest x-ray
• Strong communication skills and computer proficiency
• Compassionate, collaborative, and committed to patient-centered care

Responsibilities

• Conduct comprehensive patient evaluations and reassessments
• Develop and implement individualized therapy plans in collaboration with physicians and care teams
• Provide hands-on physical therapy treatments and monitor patient progress
• Educate patients and caregivers on therapeutic exercises and equipment use
• Supervise Physical Therapy Assistants and participate in interdisciplinary case conferences
• Maintain accurate, timely documentation in accordance with care plans and policies
