About The Position

Personal Care Partner | QMAP (Part-Time Evenings) Schedule: Friday, Saturday, Sunday | 2:00 p.m. - 10:00 p.m. Join a community where older adults thrive, we welcome their voices, and choices as this is their home. We're seeking a QMAP who believes in empowered older adults and truly collaborative care. You will: Support residents with daily personal care and ADLs (bathing, dressing, grooming, meals, mobility). Build respectful, caring relationships with residents and families. Monitor safety and well-being and promptly report changes to nursing/supervisors. Assist with medications under licensed nurse supervision, following all "five rights" and documentation procedures. Communicate and collaborate with the care team to support resident goals and preferences. You are: Compassionate, patient, and respectful of resident choice. A strong team player and communicator. Experienced in caregiving and/or medication assistance (or willing to be trained, per state law). Able to work Fri-Sun, 2p-10p consistently. Requirements

  • Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
  • Must be able to read, write and speak the English language.
  • High school or Equivalent (GED)
  • One (1) year experience in some level of elder care preferred
  • Proficient computer skills preferred
  • Strong self-management and organization skills
  • Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable (Open to change).
  • Ability to effectively handle difficult and sensitive issues and maintain confidentiality
