About The Position

Provide an ongoing program of activities designed to meet the interests and the physical, mental and psychological well-being of each patient. Discharges duties in a professional manner while demonstrating values of respect, dignity, integrity and compassion. Actions are consistent with hospital policies and procedures and current standards/regulations governing nursing services and long term care.

Requirements

  • High School diploma or equivalent
  • Obtain certification in Basic Life Support within 30 days of hire date.
  • Completion of recreation specialist training course approved by State of West Virginia within 6 months.

Responsibilities

  • Performs data entry and discharge tracking.
  • Provides care and education appropriate to the age of the patients served. (Adult 18-79 years, Geriatric 80+ years)
  • Maintains an understanding of treatment necessary for each patient and provides appropriate care based on their age specific needs.
  • Maintains communication in a professional manner including, but not limited to: Establishes and maintains good rapport with patients, families, volunteers, and peers. Provides emotional support to patient and family.
  • Attends in-services and other educational programs as required with special emphasis on programs related to the geriatric patient.
  • Attends and participates in at least three staff meetings.
  • Determines areas of self-improvement; writes developmental goals on performance appraisal yearly.
  • Provides activity in-service program for department.
  • Reviews and initials meeting minutes and postings in department.
  • Supports hospital and departmental cost containment efforts.
  • Plans, implements and evaluates activity program.
  • Provides individual and group activities which promote the maintenance to enhancement of the patient's quality of life.
  • Accurately completes specific areas of the minimum data set (MDS).
  • Develops individualized plan of care with the patient.
  • Establishes measurable goals, evaluates plan of care and adjusts plan as needed.
  • Prepares calendar of activities one month in advance : Activities vary per month with special events each month Activities are posted monthly
  • Prepares and maintains activity records and reports.
  • Transports patients to and from activities.
  • Coordinates and supervises activities of volunteers. Assists in the orientation and development of volunteers for Skilled Care. Coordinates the daily activities and duties of the volunteers.
  • Orders supplies and equipment for activity program while working within departmental budget.
  • Participates in continuous improvement activities.
  • Fosters relationships with community groups and organizations to provide activities for the skilled patient.
