About The Position

Telos Health Systems is seeking a Psychotherapist (LCMHC, LMFT, LCSW) or Psychologist, PsyD or PhD to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities throughout the Winston-Salem, NC area. This role offers flexibility, autonomy, and strong clinical and administrative support.

Requirements

• Active LCMHC, LMFT, LCSW or Psychologist, PsyD or PhD in North Carolina
• Master’s degree in counseling LCMHC, LMFT, LCSW
• Doctorate degree Psychologist, PsyD or PhD
• Ability to work independently while collaborating with a clinical team.
• Strong communication.

Nice To Haves

• Experience working with geriatric populations.
• Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.

Responsibilities

• Conduct comprehensive mental health assessments and diagnostic evaluations.
• Provide individual psychotherapy and ongoing treatment.
• Develop and implement treatment plans in collaboration with a multidisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Coordinate care with facility staff and other healthcare professionals.
• Adhere to all federal, state, and organizational standards and regulations.
