About The Position

Position is located at Minneapolis Riverside & University Children's Clinics Job Summary: Conducts diagnostic assessments under the supervised practice of fully licensed and system credentialed clinical supervisor, determines clinical interventions, facilitates therapy and provides education for patients and their families. Facilitates treatment planning and manages each case. Responsible for development of transition/discharge plan. Communicates effectively and respectfully with others, including interdisciplinary teams and all other contacts. Functions as an interdisciplinary team member. Accepts supervision and consultation with a licensed supervisor for the functions of this position in their clinical practice. We are currently hiring for the Behavioral Health Clinician (BHC) position, a key role within a multi-disciplinary team in the Integrated Behavioral Health model. We are looking for behavioral providers with a strong interest in diagnostic, clinical, team engagement, and patient care skills who enjoy innovation, variability in your work day, and being a part of an exceptional team. A Behavioral Health Clinician (BHC) is a fully integrated Mental Health Professional and core member of the primary care team that works under the direction of the primary care provider(s). The role of the BHC is to support the primary care team in the early identification and treatment of behavioral health conditions within the defined clinic population. The primary functions of the BHC are to: Complete diagnostic assessments Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in health and behavior change Support the team with the day to day management of complex patients Provide brief evidence-based direct care to patients Engage in population health activities to support risk stratified populations

Requirements

  • Master's Degree in Social Work, Psychology or closely related field with emphasis on clinical practice.
  • MN state license, eligible to be licensed in the State of Minnesota and eligibility to become a credentialed provider with insurance entities.
  • Completion of clinical education and internship as required by graduate program.
  • Be licensed by Board as LGSW.

Nice To Haves

  • Experience in providing diagnostic assessments and direct evidence-based clinical services to children and adults.
  • Experience with Electronic Health Record systems preferred.

Responsibilities

  • Demonstrates clinical competence in the treatment planning process.
  • Facilitates treatment planning and oversee management of each case.
  • Coordinates/facilitates the development of treatment plan by collaborating with patient, family and interdisciplinary team.
  • Develops an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjust treatment plan as necessary.
  • Demonstrates clinical competence to assess individual patient needs.
  • Completes an interdisciplinary assessment tool with each patient.
  • Updates interdisciplinary assessments tool each week.
  • Demonstrates clinical competence in disposition planning.
  • Interfaces with government agencies such as court, ombudsmen, county services
  • Completes referral process for next level of care
  • Communicates to other providers in the community
  • Sets up intake appointments
  • Completes needed paperwork to facilitate this process i.e. applications, letters of recommendation, etc
  • Demonstrates clinical competency to partner with multi-disciplinary team in the treatment planning process.
  • Assists in developing an individualized plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs.
  • Implements, reassesses and adjusts treatment plan as necessary.
  • Communicates frequently and effectively with interdisciplinary team to ensure continuity and coordination of care.
  • Documentation is thorough, timely, legible and clinically appropriate and meets regulatory requirements.
  • Demonstrates competence in developing a clinically relevant transition/discharge plan.
  • Collaborates with patient, family, and provides input to interdisciplinary team in developing a discharge plan
  • Develops discharge plan based on age, social and cultural considerations, clinical diagnosis and level of functioning and other relevant needs.
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Demonstrates clinical competence in conducting group therapy, family therapy and psycho-education groups.
  • Provides individual, couples and family therapy.
  • Documents thoroughly, timely, legible and clinically appropriate.
  • Demonstrates the ability to advance evidence based practice by utilizing standards, guidelines and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced.
  • Promotes a culture of evidence based practice by participation in any of, but not limited to, the following activities: Utilizing evidence based policies in clinical practice
  • Attends and participates in learning opportunities about evidence based practice
  • Membership in hospital or unit based practice council/research council
  • Evidence based practice project (e.g. policy development or poster presentation)
  • Journal club, journal article critique and circulation
  • Literature review
  • Development of standard/pathway/guideline for patient care
