About The Position
Telos Health Systems is seeking a Psychotherapist (LCSW, LPCC, LMFT, or Licensed Psychologist – PsyD/Ph.D. ) to provide mental health evaluations and psychotherapy services to patients in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.
Requirements
- Active Kentucky license as one of the following: Licensed Clinical Social Worker (LCSW) Licensed Professional Clinical Counselor (LPCC) Licensed Marriage and Family Therapist (LMFT) Licensed Psychologist - PsyD/Ph.D.
- Master’s degree (LCSW, LPCC, LMFT) or Doctoral degree (Psychologist) in a relevant mental health field.
- Ability to work independently while collaborating effectively within a clinical team.
- Strong communication and clinical documentation skills.
Nice To Haves
- Experience working with geriatric populations.
- Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings.
Responsibilities
- Conduct comprehensive mental health assessments and diagnostic evaluations.
- Provide individual psychotherapy and ongoing treatment services.
- Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team.
- Maintain accurate, timely, and compliant clinical documentation.
- Coordinate care with facility staff, medical providers, and other healthcare professionals.
- Adhere to all federal, state, and organizational standards, policies, and regulations.
