About The Position

At N.E.W. Community Clinic, our mental health team meets people where they are-with compassion, dignity, and respect. Position Summary: Responsible for providing counseling services, ensuring client access and acclimation to mental health services, providing crisis stabilization to N.E.W Community Clinic patients, and documentation/psycho-social screening support to N.E.W providers in accordance with policies and procedures of N.E.W & DHS 35 and current federal, state and local standards, guidelines and regulations.

Requirements

  • Education: Completion of master's in psychology, counseling, or social work degree through an accredited program meeting requirement for Wisconsin Certification. Obtained in-training license through the State of Wisconsin Department of Safety and Professional Services as an APSW or LPC-IT.
  • Experience: Experience in provision of an eclectic problem-solving approach to clients seeking short-term interventions and problem resolution. Familiarity with a range of issues such as poverty, community, family violence, HIV, human sexuality, AODA, criminal justice systems, chronic or terminal illness and pain, parenting skills, are desirable.
  • Expertise: Ability to provide culturally competent services to a variety of clients in a direct responsive manner. Willingness to confront traditional barriers in respect of various servicing patients with various mental health diagnosis. Strong interactive "team" skills within a primary medical care setting. Excellent interpersonal skills and empathy toward patients. Able to develop teaching and counseling sessions, as needed. Must be able to adapt to changing situations and circumstances. Must be willing and able to assume and delegate patient needs, as a member of the health care team.
  • Language: Comprehend and use basic language either written or spoken to communicate information and ideals.

Nice To Haves

  • Experience with current EPIC EMR a plus.

Responsibilities

  • Assist the case management team in providing pre-admission consultation, including screening, intake services, and other case management duties as needed.
  • Maintain an outpatient therapy caseload, and facilitate therapeutic groups as needed.
  • Provide outreach mental health services in the community as needed.
  • Conduct psychosocial intake assessments, including obtaining information about chief complaints, diagnosis, medication, as well as psychiatric history.
  • Initiate patient treatment plans by identifying needs, strengths, weaknesses, and formulating strategies for life-style improvements.
  • Effectively and accurately communicate patient services/treatment and related needs across disciplines via EPIC.
  • Assist providers with completion of clinical and legal documentation requested by N.E.W patients (i.e., social security disability forms, transportation authorizations, letters of diagnosis, etc.). Under direct supervision of N.E.W mental health practitioners, physicians, and physician assistants, psychiatric APNP.
  • Work with psychiatric patients and providers to provide best-practice education/resources, payee services, and education to essential skills of daily living as living.
  • Maintain knowledge of other community systems/resources, make external referrals for higher level of mental health care when necessary, and identify unmet needs to ensure referrals are accomplished and that quality services are available.
  • Assist clients with internal referrals to other N.E.W. service lines (i.e., case management, medical, dental, WIC).
  • Represent N.E.W at community trainings related to increasing clients' awareness to community mental health services and resources.
  • Consult with staff regarding psychosocial needs that encompass social, ethnic, religious, and cultural diversity of individual clients.
  • Participate in continuing education for certification/license and career upgrade.
  • Participate in staff meetings, intra services client staffing case management as needed.
  • Assist N.E.W providers with completion of primary care brief screenings (i.e., PHQ-9, and other required screenings).
  • Perform other job-related duties, as may be assigned.
