About The Position

The Psychiatry Registered Nurse is responsible for facilitating patient care, providing care coordination, care management, direct patient care and patient education to a diverse patient population. Serves as a liaison between Psychiatry Primary care. Responsible for thorough and accurate patient documentation, and for participating as a contributing member of the Psychiatry and greater health care team.

Requirements

  • Registered Nurse licensure
  • Good standing with the Vermont State Board of Nursing
  • Experience in Mental Health Field
  • BLS Certified

  • Case Management/Care Coordination experience a plus

Responsibilities

  • Coordinate discharge planning for CHCB patients who are being discharged from inpatient psychiatric hospitalization, partial hospitalization, and from addiction rehabilitation centers.
  • Collaborate with other members of the case management, Care team to ensure that there is continuity of care between discharge planning and case management. Work to ensure that patients who are discharged from other facilities have appointments at CHCB and that they attend the first appointments. Coordinate information about the attendance to the psychiatry, behavioral health, and primary care providers also working with the patient, or who are scheduled to work with the patient.
  • Manage a caseload of case management clients.
  • Attend case management/care coordination meetings at CHCB or in the community.
  • Perform as a resource for Mental Health population under Care management.
  • Facilitates patient care activities and coordinates flow of patients in the clinical setting.
  • Consults and coordinates with Psychiatry and other health care team members to assess, plan and implement and evaluate patient care plans.
  • Assists in the training and supervision of new nursing staff, nursing students and other medical service-related personnel, as needed.
