About The Position
The Alternative Community Resource Program is seeking a Psychiatric Registered Nurse for our Psychiatric Outpatient program and Blended Case Management program. Responsibilities include: Advocates for the health of the consumer by: Referring, and facilitating access to quality physical and behavioral health services intended to meet individualized needs Facilitating access and coordination of behavioral and physical health care services Being responsive to the needs expressed by the consumers themselves; Assist and coach clients to advocate for themselves in meetings with health care providers; Develop and implement programs around compliance to medication and medical appointments; Develop wellness materials for clients on identifying and managing chronic health conditions; Develop preventative wellness plans for clients new to the system to preclude the development of chronic health conditions i.e. weight management, nutrition, health screenings, exercise, and smoking cessation; Conduct medication management appointments Check Client Vitals Complete Prior Authorizations Monitor clients enrolled in the Wellness Program.
Requirements
- Registered nursing license in the state of Pennsylvania
- At least of one year of nursing experience
- At least one year experience in mental health and/or drug and alcohol abuse
- PA State Police Clearance
- Act 31 ChildLine Clearance
- FBI Clearance
- Act 31 Certificate
- Valid PA Driver's License with use of reliable transportation
