You will provide essential psychiatric care to elderly residents in skilled nursing, rehabilitation, and long -term care communities — patients who often face limited access to consistent behavioral health support. Requirements
- Board -certified Psychiatric Nurse Practitioner
- Active, unrestricted state license
- Current DEA registration
- Minimum 2+ years of experience working with geriatric or long -term care populations
- Advanced knowledge of dementia -related behavioral management
- Confidence managing medically complex, polypharmacy -heavy patients
- Ability to function independently while contributing to a collaborative care model
Responsibilities
- Perform comprehensive psychiatric evaluations and longitudinal follow -up care
- Treat mood disorders, psychosis, anxiety disorders, trauma -related conditions, and neurocognitive decline
- Manage Behavioral and Psychological Symptoms of Dementia (BPSD) using evidence -based interventions
- Optimize medication regimens while minimizing polypharmacy and adverse effects
- Conduct capacity assessments and behavioral risk evaluations
- Collaborate closely with nursing leadership, medical teams, and social services
- Provide staff education and guidance to strengthen behavioral health practices within facilities
