You will provide essential psychiatric care to elderly residents in skilled nursing, rehabilitation, and long-term care communities — patients who often face limited access to consistent behavioral health support. Our providers make a measurable difference by stabilizing complex psychiatric conditions, reducing unnecessary hospital transfers, addressing behavioral disturbances with dignity, and improving overall quality of life for residents and their families. In this role, you will: ◆ Perform comprehensive psychiatric evaluations and longitudinal follow-up care ◆ Treat mood disorders, psychosis, anxiety disorders, trauma-related conditions, and neurocognitive decline ◆ Manage Behavioral and Psychological Symptoms of Dementia (BPSD) using evidence-based interventions ◆ Optimize medication regimens while minimizing polypharmacy and adverse effects ◆ Conduct capacity assessments and behavioral risk evaluations ◆ Collaborate closely with nursing leadership, medical teams, and social services ◆ Provide staff education and guidance to strengthen behavioral health practices within facilities

Requirements

  • Board-certified Psychiatric Nurse Practitioner
  • Active, unrestricted state license
  • Current DEA registration
  • Minimum 2+ years of experience working with geriatric or long-term care populations
  • Advanced knowledge of dementia-related behavioral management
  • Confidence managing medically complex, polypharmacy-heavy patients
  • Ability to function independently while contributing to a collaborative care model
