Position Overview I. SCOPE OF ROLE: The Psychiatric Nurse Practitioner will be an integral part of a dynamic team of high caliber, passionate professionals working to advance and improve healthcare practices and outcomes. Working collaboratively with the ACT Team and Medical Director, they will help implement interventions while continuously assessing / evaluating to improve overall psychiatric health care delivery. The Psychiatric Nurse Practitioner will support exceptional care for each individual through the use of an evidenced-based approach and best practices to achieve optimal quality and person-centered outcomes. II. ESSENTIAL DUTIES & RESPONSIBILITIES: Meet with Program Director, Program Nurse, and ACT staff to coordinate ongoing psychiatric treatment of clients on ACT Team. Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Review health assessments completed by the Program Nurse and provide clinical feedback. Provide medication management including prescription refills and medication education to consumers. Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes. Prepare Psychiatric Assessments bi-annually and for other purposes such as for HRA 2010e Housing Applications and/or other applications for housing or needed services. Provide training to ACT team members on topics including medications and drug interactions. Communicates and coordinates care with psychiatrists in the community, as needed. Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff. Provide in service staff training as needed and consultation as needed to the ACT team. Provide crisis intervention and address all emergency situations as appropriate. Maintain up to date, accurate, complete, and timely consumer chart documentation. Attends daily morning meetings, staff meetings, case conferences, group supervision, and trainings as needed. Attend supervision with Medical Director at least monthly. Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, Rules, Regulations, and Statutes.
