The psychiatric nurse practitioner (PNP) will diagnose and treat patients with mental, emotional, substance use, and behavioral disorders in an interdisciplinary, integrated care setting. Position focuses on psychotropic medication management of patients and education of interdisciplinary colleagues on psychotropic prescription. The PNP will participate in programmatic design of integrated health systems and offer guidance in the integration of psychiatry. The PNP will order laboratory and diagnostic tests as indicated, and maintain individualized treatment plans for patients. The PNP coordinates patient care utilizing a variety pharmacotherapy, internal referrals and linkages to appropriate community resources. The PNP works with the behavioral health team to coordinate therapy and psychiatric services, and collaborates with primary care providers on shared patients.

Requirements

  • Licensed as an Advance Practice Nurse in the state of Illinois with DEA Controlled Substance license and Master’s degree in Nursing

Responsibilities

  • Complete psychotropic medication evaluations and provide medication management follow up for Howard Brown Health (HBH) primary care patients.
  • Provide diagnoses, prescriptions, and treatment plans in collaboration with HBH primary care or behavioral health providers.
  • Communicate effectively with medical and behavioral health providers to ensure continuity of care.
  • Perform psychiatric visits with the Psychiatry Mobile Unit once weekly.
  • Administer injectable psychotropic medications.
  • Offer periodic trainings to HBH primary care and behavioral health providers on psychotropic medication use and prescription.
  • Participate in programmatic design of new and existing integrated care settings, offering guidance to the Director of Behavioral Health and the Associate Director of Psychiatry.
  • Collaborate with Behavioral Health Providers in the primary care teams to develop workflow and consult as necessary on patient care.
  • Monitor effectiveness of pharmacotherapy and provide intervention on an as-needed basis.
  • When clinically appropriate, return patients to primary care providers’ care for ongoing prescription and treatment.
  • Document patient contact/progress in electronic health record system (EHRS).
  • Develop and maintain required forms, reports, and data in a timely manner and in accordance with policy and procedures.
  • If critical, consult with HBH primary care and behavioral health providers when off-site.
  • Offer primary care providers consultation regarding management and prescription of psychotropic medications.
  • Produce/complete necessary documentation for patients including social security determination documents and patient-related forms and letters, such as state ID or public transportation documentation, and letters of safe passage.
  • As agreed to by patients, collaborate with patients’ families and external support staff and case managers to improve treatment outcomes and ensure continuity of care.
  • Meet with the Associate Director of Psychiatry, Director of Behavioral Health or the Chief Behavioral Health Officer as scheduled.
  • Complete all requirements for meaningful use and credentialing/staff privileges, and comply with all medical staff training requirements including annual HIPAA training, risk management training, and employee health screening. These requirements are determined by HBH and by any affiliated hospitals or medical staff groups as determined by HBH.
  • Comply with appointment policies and procedures, including policies regarding late appointments, no show practices, and time allotted for new and return appointments.
  • Adhere to standards of practice and ethical guidelines established by the American Psychiatric Association, the State of Illinois, and HBH.
  • Maintain all appropriate licenses and privileges as determined by Chief Behavioral Health Officer.
  • Make program recommendations to the Medical and Behavioral Health Directors, assisting in the development of policies and procedures as needed.
  • Other duties as assigned.
