About The Position

Requirements

  • U.S. Citizenship is required.
  • Must possess a full and unrestricted license from any U.S. State or territory.
  • Must have a post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
  • Maintain full and current board certification in an area of clinical specialty from a recognized certifying body.
  • Proficiency in basic written and spoken English is required.

Responsibilities

  • Independently manage a panel of patients with acute and chronic health conditions.
  • Develop and implement individualized plans for optimal population health and disease management.
  • Demonstrate leadership and collaborate with the interdisciplinary team.
  • Order and interpret labs, tests, and diagnostic studies.
  • Prescribe pharmacologic and non-pharmacologic interventions.
  • Initiate referrals and consults to specialty services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.
