The Psychiatric Nurse Practitioner (PMHNP) serves as a core member of the primary care and behavioral health team, delivering psychiatric expertise within an integrated, team-based care model. The PMHNP supports primary care providers (PCPs), care managers, social workers, and behavioral health staff by providing timely assessments, medication management, consultation, and collaborative treatment planning. This role ensures patients receive accessible, coordinated, whole-person mental health care directly within the primary care setting.

Requirements

  • Master's or Doctorate degree in Nursing with PMHNP certification.
  • Active Utah APRN license (or ability to obtain before start date).
  • DEA license with appropriate prescribing authority.
  • Experience or strong interest in working as part of a primary care and behavioral health team.

  • Experience in integrated or collaborative primary care settings.
  • Familiarity with the Collaborative Care Model or similar team-based approaches.
  • Training or experience in trauma-informed care, motivational interviewing, or brief therapeutic interventions.

  • Proficiency in diagnosing and treating common psychiatric conditions in medical settings.

Responsibilities

  • Provide psychiatric evaluations for patients referred by members of the primary care team.
  • Deliver medication management for common mental health conditions encountered in primary care.
  • Offer brief, supportive clinical interventions aligned with integrated behavioral health workflows.
  • Participate in warm hand-offs and joint visits to improve patient engagement.
  • Act as a psychiatric consultant and clinical partner to primary care providers and behavioral health team members.
  • Participate in daily/weekly huddles, case reviews, and Collaborative Care-style meetings with the care team.
  • Coordinate closely with care managers, nurses, and social workers to ensure follow-up, safety planning, and referrals.
  • Support shared decision-making and unified treatment plans across the care team.
  • Maintain clear and timely communication with primary care and behavioral health team members regarding clinical findings, treatment plans, and patient progress.
  • Facilitate referrals to specialty psychiatric services when required.
  • Assist with transitions of care, including hospital discharges involving behavioral health needs.
  • Use standardized screening tools (PHQ-9, GAD-7, ADHD scales) to guide diagnosis and monitor outcomes.
  • Support Revere Health's value-based care goals through improved access, coordinated care, and enhanced patient outcomes.
  • Maintain accurate clinical documentation that supports integrated primary care and behavioral health workflows.
