About The Position
Mission Connection extends the compassion-driven, evidence-based clinical care of AMFM Healthcare through an innovative hybrid model that blends in-person and telehealth services. Our program provides comprehensive mental health support for individuals experiencing primary mental health diagnoses such as anxiety, depression, and mood disorders. Mission Connection empowers clients to connect with their clinical team when, where, and how it’s most convenient — ensuring accessible, high-quality care that truly meets them where they are. We are currently seeking a Licensed Practical Nurse (LPN) licensed in Virginia to join our remote clinical team. The LPN will provide essential nursing support services in a fully remote capacity, collaborating closely with our multidisciplinary behavioral health team. This role is ideal for a detail-oriented, compassionate nurse who thrives in a structured telehealth environment and values high clinical standards. This is a part-time remote position, Monday through Friday from 9:00 AM – 1:00 PM EST. (6:00 AM - 10:00 AM PST)
Requirements
- Active and unrestricted Virginia LPN license (required)
- Must be 21 years of age or older
- CPR/First Aid Certification
- Successful completion of required background screenings
Responsibilities
- Observe, assess, and document client conditions; report changes appropriately
- Manage prescribed medications and accurately document administration and monitoring
- Record and maintain vital health information as appropriate in a telehealth setting
- Maintain integrity and accuracy of all documentation within the Electronic Health Record (EHR) system
- Ensure timely scanning, uploading, and auditing of medical documentation
- Address Medication Incident Reports promptly and accurately
- Support policies related to Medication Management and Infection Prevention & Control
- Uphold National Patient Safety Goals and clinical best practices
- Coordinate care with the clinical team to assess, plan, and implement appropriate interventions
- Maintain strict HIPAA compliance and patient confidentiality
- Assist with administrative and clerical tasks as needed
