Role Overview: Part‑Time PMHNP — Hampton, VA We are seeking a compassionate and experienced PMHNP to provide part‑time psychiatric assessment, medication management and psychotherapy services to residents in post‑acute, long‑term care and affiliated outpatient settings in Hampton. The successful clinician will apply strong clinical judgment in managing psychiatric and behavioral health conditions, collaborate closely with facility teams, and contribute to person-centered care plans that promote stability, function and quality of life.

Requirements

  • Master’s or Doctoral degree as a Psychiatric‑Mental Health Nurse Practitioner from an accredited institution
  • Current and unrestricted Virginia Nurse Practitioner license with PMHNP certification and any required state credentials
  • Current DEA registration as required for scope of practice
  • Proficiency with Electronic Medical Record systems and accurate clinical documentation
  • Strong clinical judgement, excellent communication skills and the ability to work collaboratively within multidisciplinary teams
  • Commitment to compassionate, person‑centered care, cultural sensitivity and ethical practice
  • Current Basic Life Support (BLS) certification required; additional certifications (e.g., crisis intervention, advanced behavioral management) desirable

  • Experience working in post‑acute, long‑term care, geriatric psychiatry or community behavioural health settings preferred

Responsibilities

  • Provide psychiatric evaluations, diagnostics and ongoing medication management for residents with mood, anxiety, psychotic and neurocognitive disorders
  • Deliver brief psychotherapeutic interventions and crisis management appropriate to the post‑acute setting
  • Collaborate with nursing, social work and interdisciplinary teams to develop individualized, measurable care plans focused on reducing symptoms, minimizing hospitalizations and optimizing quality of life
  • Perform timely review and reconciliation of psychotropic medications, monitor efficacy and side effects, and engage in deprescribing when clinically appropriate
  • Order, interpret and act upon relevant diagnostic tests and specialist consults; coordinate referrals to behavioural health services as needed
  • Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
  • Participate in facility rounds, care‑planning meetings and family conferences to ensure coordinated, person‑centred behavioural health care
  • Support facility quality and safety initiatives related to behavioural health, falls reduction, deprescribing and behavioural management
  • Provide clinical education and guidance to facility staff on management of psychiatric symptoms, non‑pharmacologic interventions and trauma‑informed care
