About The Position

We are expanding our behavioral health services and are seeking a licensed Psychiatric Nurse Practitioner to provide ongoing psychiatric care across affiliated post -acute and residential facilities. This role is well suited for a clinician who values autonomy, consistency, and the opportunity to build long -term therapeutic relationships outside of hospital environments. The position offers a predictable schedule, strong clinical support, and the ability to practice independently within a collaborative care model.

Requirements

• Board -certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Active, unrestricted state license
• Strong clinical judgment and independent decision -making skills
• Excellent interpersonal and professional communication abilities

Nice To Haves

• Background in skilled nursing, assisted living, or geriatric psychiatry settings
• Prior experience in residential or institutional behavioral health environments

Responsibilities

• Conduct initial psychiatric evaluations and manage longitudinal follow -up care
• Provide medication management and monitor treatment effectiveness
• Develop individualized care strategies for patients with chronic psychiatric conditions
• Coordinate closely with physicians, nursing staff, social workers, and facility leadership
• Educate patients and families on diagnoses, medications, and treatment plans
• Deliver evidence -based, patient -centered psychiatric care
• Maintain accurate, timely documentation in accordance with regulatory standards
