Requirements
- Current licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP or PMHNP-BC).
- A Valid DEA Number
Responsibilities
- Conduct comprehensive psychiatric evaluations and mental health assessments for patients in Long Term Care facilities, ensuring continuity of care.
- Develop and implement individualized treatment plans, including prescribing and managing psychotropic medications.
- Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes.
- Monitor and manage chronic mental health conditions such as depression, anxiety, bipolar disorder, and schizophrenia.
- Maintain accurate documentation of patient visits, treatment plans, and progress notes using electronic medical records (EMR).
