About The Position

Amae Health is a Series B, venture-backed Public Benefit Corporation that delivers specialty outpatient care for people with severe mental illness. We build and operate high-acuity, in-person clinics nationally. Our model integrates psychiatry, therapy, primary care, dietitians, and community support under one roof. It has proven itself as the most effective in the country, which is why we collaborate with leading health systems including NewYork-Presbyterian, Cedars-Sinai, Mass General Brigham, Novant Health, and CommonSpirit to extend our care to where it’s needed most. But what makes us fundamentally different is that we are also building the science and technology layer that this field has never had. We are developing proprietary clinical protocols, designing novel measurement science, and engineering an AI data science platform purpose-built for the complexity of severe mental illness. We are hiring a Psychiatric Mental Health Nurse Practitioner (PMHNP) (full- or part-time) to join our growing Charlotte, NC clinic. You’ll be part of an interdisciplinary team dedicated to delivering personalized, compassionate, and high-quality care for individuals with serious mental illness (SMI). As a Psychiatric Mental Health Nurse Practitioner at Amae, you will play an integral role in our collective mission to become the center of excellence for SMI care. In alignment with Amae’s holistic wrap-around care model, you will co-manage a panel of complex members through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning. Working within a collaborative and shared decision-making model, you will partner closely with psychiatrists, therapists, peer specialists, primary care providers, and community partners. As a patient advocate, you will help members navigate the health and social services systems in Charlotte and work closely with patients to address problems, answer questions, gather and provide information and advice, and connect patients to resources and community supports. Rooted in trust and rapport, you will initiate, establish, and maintain relationships not only with patients but also with families as you care for members of Amae Health. Requirements

  • Master’s or Doctoral degree in Nursing from an accredited program
  • Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
  • Active, unrestricted North Carolina NP license and prescriptive authority in good standing
  • DEA registration (or ability to obtain)
  • In possession of or willingness to obtain Clozapine REMS certification and Medicaid enrollment, if needed
  • Prior experience working with individuals with serious mental illness and/or substance use disorders
  • Familiarity with the medical, mental health, and social service systems in the greater Charlotte area
  • A holistic perspective on “health” and an understanding of patient rights
  • Demonstrated ability and desire to work effectively with people from diverse backgrounds, including patients and colleagues
  • Comfort working in both one-on-one and interdisciplinary team settings, with strong coaching and communication skills
  • Excellent interpersonal and communication skills and the ability to adapt in a dynamic environment
  • Comfort with technology and a passion for using technology to improve care delivery and patient outcomes

Responsibilities

  • Provide behavioral healthcare management to Amae Health patients, monitoring and managing care in alignment with a holistic approach and applying an array of treatment modalities as patients progress through phases of recovery
  • Conduct psychiatric evaluations and assessments and provide ongoing medication management for patients with serious mental illness
  • Develop individualized treatment plans early during the engagement and stabilization phase, focused on addressing specific psychosocial and medical needs that might limit recovery
  • Provide continuous, regular consultations as patients navigate recovery, both in person and occasionally virtually
  • Liaise with patient care teams and collaborate closely with behavioral health providers to ensure an integrated, cohesive approach to care
  • Address medical and behavioral health needs to mitigate the negative impacts of social determinants of health
  • Educate patients and families about mental health conditions, treatment options, and recovery planning
