SOME (So Others Might Eat) exists to help homeless and poor individuals and families in Washington, DC. Our more than 30 programs throughout the district provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare, and offer the tools one needs to live with hope, dignity, and greater independence. The Nurse Practitioner (NP) Contractor provides high-quality, integrated primary and behavioral health care to individuals experiencing homelessness and/or severe poverty. The NP will deliver direct clinical services, coordinate interdisciplinary care, and support compliance and quality improvement initiatives in alignment with SOMEs mission and Federally Qualified Health Center (FQHC) standards.

  • Provide outpatient clinical services including assessment, diagnosis, treatment, and referral to specialists as needed.
  • Deliver emergency care when appropriate and document per accepted protocols.
  • Oversee disease prevention and health promotion protocols in line with established standards.
  • Collaborate with the Medical Director, Senior Director, Senior VP, clinical staff, and volunteers in daily clinical operations.
  • Review abnormal laboratory/test results and make recommendations.
  • Assess, diagnose, and treat co-occurring medical, psychiatric, and substance use disorders in collaboration with the integrated care team.
  • Provide crisis intervention, stabilization, and linkage to higher levels of care within scope of practice.
  • Collaborate with primary care providers, behavioral health consultants, case managers, SUD counselors, and interdisciplinary team members.
  • Coordinate care integrating medical, psychiatric, behavioral health, and substance use treatment.
  • Facilitate referrals to higher levels of care as appropriate.
  • Participate in case conferencing and interdisciplinary huddles.
  • Complete timely and accurate EHR documentation consistent with FQHC, HRSA, and payer requirements.
  • Ensure compliance with HIPAA, 42 CFR Part 2, and other applicable regulations.
  • Support documentation for billable services under Medicaid/Medicare and other payer guidelines.
  • Document trauma-informed, recovery-oriented integrated care plans.
  • Provide care using trauma-informed, culturally responsive, and recovery-oriented approaches.
  • Apply harm reduction principles with patients experiencing substance use disorders.
  • Support outreach-based care delivery, including walk-in and same-day appointments.
  • Foster engagement and trust with unstably housed or unhoused patients.
  • Provide consultation and education to SOME staff and departments to support client care.
  • Deliver psychoeducation to housing, SUD, and primary care teams to promote integrated care and early intervention.
  • Maintain active licensure, DEA registration, board certification, and malpractice insurance.
  • Participate in supervision, training, and quality improvement initiatives.
  • Adhere to professional and ethical guidelines for Nurse Practitioners.
  • Contribute to FQHC-wide quality improvement efforts to reduce health disparities.
  • Active licensure as a Nurse Practitioner in Washington, DC.
  • DEA registration and board certification.
  • Malpractice insurance coverage.
  • Experience in providing integrated primary and behavioral health care.
  • Knowledge of FQHC standards and compliance requirements.
  • Experience working with homeless or low-income populations.
  • Familiarity with trauma-informed care principles.
  • Experience in substance use disorder treatment.
  • Competitive compensation of $90 - $95 per hour, commensurate with experience.
  • Assistance with credentialing for DC Medicaid and MCO providers.
