The Psychiatric Mental Health Nurse Practitioner (PMHNP) - School Based (Warren Township) delivers comprehensive, evidence-based behavioral health services within a collaborative, team-based model. This role is responsible for assessing, diagnosing, and treating patients with mental health concerns who are being seen in a school environment. The PMHNP works closely with primary care providers, therapists, school leadership, and care teams to support whole-person health, reduce stigma, and improve access to mental health services within the medical home. Providers will have to float between sites and between school districts to cover for other providers when appropriate.
