Requirements

• Dual certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) and Family Nurse Practitioner (FNP) is mandatory.
• Master's or Doctoral degree in Nursing from an accredited institution.
• Current and valid Texas licensure as a Nurse Practitioner.
• Certification from a recognized credentialing body (e.g., ANCC, AANP).
• Minimum of two years of experience in psychiatric and family practice settings.
• Strong clinical skills in psychiatric assessment, diagnosis, and treatment, as well as chronic care management.
• Excellent interpersonal and communication skills.
• Compassionate and patient-centered approach to care.
• Proficiency in electronic health records (EHR) systems.

Responsibilities

• Conduct comprehensive psychiatric evaluations and provide ongoing psychiatric care and management for patients via telehealth four days per week.
• Conduct chronic care management and behavioral health management for residents in a senior living community one day per week in-person.
• Develop and implement individualized treatment plans for patients with mental health and substance use disorders.
• Provide medication management for patients.
• Collaborate with a multidisciplinary team to ensure coordinated and comprehensive care.
• Perform routine health assessments, diagnose and treat common health conditions, and provide preventive care.
• Educate patients and their families about chronic disease management and healthy lifestyle choices.
• Maintain accurate and up-to-date medical records in compliance with regulatory standards.
