About The Position

The Psychiatric Nurse Practitioner provides medical care to all age groups in accordance with the State of North Carolina and of Licensing and professional scope of practice. In addition, the Nurse provides diagnostic and therapeutic psychiatric services to Pride in North Carolina, LLC., consumers/ patients, while making reasonable and sound Psychological/psychiatric assessments and treatment plans. They must work closely with the Agency Medical Director, consumer’s natural supports, other clinical staff, and community agencies. This position requires the ability to work in a mutually respectful partnership with persons served and their families to gain the skills and confidence to address any issues and problems they face. They are required to attend and participate in meetings on behalf of the company and the consumer, conduct weekly supervisory meetings with the team, and participate in clinical training events and meetings as required by the Clinical Director and/or Medical Director. Assessment and Planning: Works with directly with Medical Director, Lead Clinician and Regional Director/Program Coordinator to review new intakes and appropriateness for Services. Assesses strengths, weaknesses and mental condition through ongoing completion of assessment tools with consumer and family. Uses assessment information to develop a clinical profile and provide guidance to any updates to the Person Centered Plan. Develops transition plans, and follows up with consumers after any unplanned discharges to inquire on their status and/or if they are in need of additional services. Relationships with Consumers and Families: Ensures that therapeutic interactions are sensitive to and in accordance with the family’s cultural needs and interests. Provides information to the consumer or patient and family regarding their rights, services, and the formal grievance process as needed. Respects and maintains confidentiality with regard to information concerning consumer, parents/guardians, and other family member Documentation: Documents in consumer records in accordance with Service Records Requirements and Medicaid guidelines. Ensures that all data from all agency staff is entered into the information management system and is accurate, complete, and entered in a timely manner. Service Implementation: Provides psychiatric care, including assessment, testing, medication prescribing and monitoring all age groups in accordance with the State of North Carolina and professional scope of practice. Diagnose the nature and extent of mental and/or substance-related disorders by reviewing the client’s CSC clinical record, requesting and reviewing known medical/psychiatric documentation, coordination with other providers, and evaluating the client's current physical and psychological condition. Counsels family members to assist in understanding, dealing with, and supporting consumer. Provides support and assistance on psychiatric issues to primary care providers (PCP) and agency staff and other stakeholders. Makes recommendations relative to psychiatric strategies and medications, and conducts follow-up consultations to reinforce psychiatric interventions. Monitors, evaluates and records consumer progress according to goals in Person Centered Plan. Ensures that treatment is based on strengths, needs, abilities, and preferences. Supports the efforts of Pride in North Carolina, LLC. staff to assess and triage consumers for levels of psychological functioning and risk, determines need for referral to Psychiatrist or another agency, and assists with psychological diagnosis and management. Performs follow-up psychiatric behavioral health consultations by performing psychiatric assessments, recording findings in patient charts, developing behavioral care plans with agency staff, assisting consumers with compliance with planned interventions and medications, maintaining consumer stability/preventing relapses, assisting with difficult conditions, procedures or lifestyle changes, and assisting in managing disability issues. Establish and maintain practice standards and protocols for medical staff. Assure compliance with all related licensing, DEA, and other regulations. When needed, conducts group classes to promote skills-building and education, provide disease information, propose interventions, offer assistance in coping with side effects, and address negative believes about interventions. Serves as liaison to community specialty behavioral health providers by identifying beneficial community resources and making referrals, educating consumers about external resources, and serving as liaison with those providers to assure effective communication. Advocates for consumers with outside agencies when they are require services beyond the scope of Pride in North Carolina, LLC. Crisis Management: In conjunction with the team develops a comprehensive crisis plan. Reviews and modifies the crisis plan as needed based on psychiatric symptoms. Is available 24 hours a day, seven days a week to handle emergencies. Ensures appropriate documentation and notifications take place following all crisis situations. Fiscal Responsibilities: Ensures that correct billing codes are documented and submits accurate billing information in a timely manner. Ensures all Medicaid/insurance information is obtained and submitted to the Corporate Office billing personnel so services can be billed. Responsible for meeting productivity requirements. Reviews all billing of services provided in the information management system, ensures that all services are accounted for and billed properly. Ensure all required billing elements are in place prior to providing services. Ensures that services provided are only services that are authorized and billable. ​​​​​​​ Supervision and Service Monitoring: Conducts meetings with agency staff as needed to provide supervision, monitor implementation of the Person Centered Plan, promote consumer stability and recovery, and monitor the overall consumer response to treatment. Monitors the own job performance and documentation to ensure compliance with Medicaid service definition requirements. Reviews documentation and ensures all notes are submitted within a timely manner, meet Medicaid requirements, and address the goals as written in the Person Centered Plan. Ensures notes are reviewed and approved within 24 hours of entry.

Requirements

  • Persons in this position are required, at a minimum, to have a Master’s Degree in a Nursing and licensure by the state of North Carolina.
