About The Position

The Psych Social Worker at Project HOME is responsible for coordinating care within an integrated medical team to enhance patient health outcomes and satisfaction. This role involves providing care coordination, assisting patients in overcoming barriers to accessing health services, and ensuring comprehensive behavioral health support. The position also includes clinical supervision that contributes to the credentialing process for LCSW/LPC.

Requirements

  • Master's Degree in Social Work
  • Competency in providing cognitive and behavioral interventions in a primary care setting
  • Experience working within trauma-informed, harm reduction, and social justice frameworks.

Responsibilities

  • Provide care coordination by assisting patients to overcome concrete and emotional barriers in connecting to needed physical and comprehensive behavioral health services.
  • Complete needed forms to connect patients to additional supports and follow up with referrals.
  • Collect mental health treatment histories and perform appropriate screeners for patients.
  • Meet with patients to support Behavioral Health services with an emphasis on trauma-informed care.
  • Identify, assess readiness, refer, and advocate for patients needing specialty behavioral health services.
  • Consult with an integrated medical team to enhance understanding of the patient and assist in treatment planning.
  • Work closely with Community Health Workers and Providers to develop collaborative care plans for patients.
  • Coordinate CBH-funded initiatives and attend community events to drive expansion of behavioral health screenings.
  • Manage Early Intervention referrals in partnership with the Reproductive and Child Health specialist team.
  • Support MOUD programming and participate in group facilitation as needed.
  • Complete all charting as required by contracted funding sources, including assessments and progress notes.
  • Participate in planned supervision and case consultation.
