About The Position

The Social Worker provides the full spectrum of social services to all patients, their families and significant others, who receive services. This includes psychosocial evaluation, discharge planning, some groups, and individual/family consultations. These services are provided by the social worker in collaboration with the interdisciplinary team.

Requirements

  • Master’s Degree in Social Work from an accredited college.
  • At least one year of previous work experience in a hospital setting and/or a psychiatric population setting.
  • Eligibility for licensure as a Social Worker.
  • Ability to safely negotiate physical environment; understand and translate written and auditory communications safely.

Responsibilities

  • The Social Worker’s primary responsibility is to facilitate the effective delivery of social services by developing and maintaining effective communications with patients, families, the multidisciplinary treatment team and referral resources.
  • As a patient advocate, the Social Worker designs the discharge plan with the patient and/or care provider and promotes the best possible plan upon discharge including aftercare referrals.
  • The Social Worker demonstrates clinical competency in behavioral health theories and practices through the implementation of psychosocial services to patients and their families, including the utilization of appropriate community resources.
  • Through interviews with patients, families, significant others, and community providers the Social Worker completes psychosocial assessments on all assigned patients.
  • The Social Worker provides a variety of therapeutic modalities including running groups, consultation to patients and their families, and other interventions as appropriate.
  • He/She collaborates with members of the multidisciplinary team during treatment planning and utilization review meetings, initiating and designing appropriate social services interventions.
  • The Social Worker formulates a plan for the timely discharge of each patient, including identification and documentation of aftercare services as needed. This process includes locating resources for placement (including out of home placement), making referrals, interfacing with community agencies and families, coordinating continuity of care as much as possible.
  • Social Workers have a dual-role as patient advocate and mental health provider. The Social Worker interfaces with community providers on behalf of the patient, yet also prepares and presents the patient’s medical records for judicial review at Probable Cause Hearings.
  • The Social Worker must balance sensitivity to individual patient needs and preferences with social service standards for safe, appropriate discharge planning.
  • The Social Worker maintains effective professional communications in both verbal and written form with all department administrators, medical staff, employees, patients, families and supervisors.
  • Assists with the accurate, legal, and timely completion of assessments, reports, forms, and documents, and at times serves as a liaison between patients, staff, physicians, community entities utilizing principles of quality customer service.
  • The Social Worker consistently works independently with maximum productivity, demonstrating problem-solving, decision-making, effective time-management skills, and self-direction.
  • Coordinates tasks to achieve maximum productivity and efficiency; performs duties independently with little or no direct supervision; shows initiative in completing tasks; maintains good attendance and keeps all required documentation current.
